Our office is a participating provider for approximately 5000 different insurance plans, including those sponsored by many of the nationally recognized insurance companies as well as plans managed by locally based employers.  Many of these plans provide for discounted exam fees, however, some plans include "funded benefits" that will pay a portion of your charges directly to the provider.  As a participating provider, we are able to bill your insurance company directly for any "funded benefits" you may be eligible for.  Cole Managed Vision, Davis Vision, Superior Vision, Aetna, Vision One, Vision Benefits of America and Medicare are only a few of the the carriers that we can bill directly.  In order for us to bill your insurance company directly, we must have authorization of eligibility before the examination is performed. 

If we are not a participating provider for your particular plan, we will make it easy for you to file for reimbursement by giving you a completed claim form that you may submit for reimbursement.  All you will need to do is sign the form and mail it to your insurance carrier.
